Young adult has attained 18 years of age while under an order of foster care placement by the juvenile court.

Young adult is in foster care under the placement and care responsibility of the County Welfare Department, County Probation Department, or Indian tribe that entered into an agreement pursuant to Section 10553.1.

Young adult is participation in a transitional independent living case plan pursuant to Section 475(8) of the federal Social Security Act (42 U.S.C. Sec 675 (8), as contained in the federal Fostering Connections to Success and Increasing Adoption Act of 2008 (Public Lay 110-351), as described in Welfare and Institutions code Section 11403.

    Fully furnished apartment with everything you need to get started. Bedroom, living room kitchen and bathroom furniture are all provided. All kitchen appliances as well as pots, pans, and utensils provided.

    Rent and utilities covered

  • Living

    By-monthly allowance. $150 for groceries and $150 for miscellaneous twice a month on the 1st and the 15th, equaling $600 per month.

What do you need to do to take advantage of AB 12?

Your Social Worker (CSW) or Deputy Probation Officer (DPO) can give you all the details. However, to be eligible you must participate in at least one of the following:

  • Completing high school or equivalent

  • Attending college with at least half-time enrollment

  • Working at least 80 hours a month (paid employment)

  • Participating in a program to obtain employment

  • Meet with your CWS/DPO once a month

  • Sign a mutual agreement and participate in your case plan

  • Agree to continued court supervision

Education, Employment & Life Skills/Mental Health

  • Education Support

    PTHP has a full time Education Specialist whose job it is to help you accomplish your educational goals. Whether completing your high school diploma or entering college, PTHP staff will help guide you through the process of enrollment, financial aid, and class selection.

  • Employment Support

    If your plan is to get started right away on your career then PTHP Career/Employment Specialist will help you with building a resume, job interview skills, and work ethics. We also help get you connected with community-based employment agencies like Proteus and C-Set.

  • Life Skills / Mental Heath

    PTHP provides life skills support in money/bill management, time management, grocery shopping, and housekeeping. PTHP also offers Mental Health and substance abuse counseling.

What kind of support and services do I receive if I chose Phoenix Transitional Housing Plus?

PTHP provides support and consistent case management services to young adults 18 to 21+ years of age needing assistance with housing, employment, education, and life skills for successful transition into adulthood.

PTHP is prepared to provide three housing options; Scattered/Staff Present Site Model, Scattered-Site (Remote) Model or the Host Family Model depending on the County’s preference and best fit for the NMD.
