Licensed by the State of California, Community Care Licensing in 2015 to provide AB-12 (age 18-21) and AB-12+ (age 21-25) services in Tulare, Fresno, Kings, Kern and Madera County.

  • Relationship-based case management is a core component of the services provided at Phoenix Transition Housing Plus.   • At Phoenix Transition Housing Plus, we offer comprehensive mental health services to support our residents in managing their mental well-being. Our team of experienced professionals provides individual therapy sessions, group counseling, and mental health assessments to create personalized treatment plans. We are dedicated to helping our residents develop coping strategies, enhance their emotional resilience, and improve their overall mental health.   • The Services are designed to promote treatment and maintain recovery from alcohol or drug problems which includes one or more of the following: detoxification, group sessions, individual sessions, educational sessions, and/or alcoholism or drug abuse. No judgement just caring and committed service.

At Phoenix Transition Housing Plus, we understand the importance of fostering these skills to empower our clients to lead independent and fulfilling lives.   • The Financial Aid Specialists provides extensive financial aid services to our clients. They delivers financial aid outreach sessions to explain the different sources of aid related to the different programs available to them.

  • Housing specialists at Phoenix Transition assist residents in finding safe housing by assessing needs, identifying options, and guiding the application process. They work with landlords and agencies to secure accommodations, supporting residents' transition to independent living with expertise and compamodations, supporting residents'ssion.

Placement Eligibility

A young adult must be on the day of their 18th birthday under the supervision of their County Welfare Department, County Probation Department, or Indian Tribe
